Our office is considered “In Network” by the following health insurance companies:
BlueCross/BlueShield
Medicare
Medicaid,
United Health
Aetna
If you carry one of the above insurances, your expenses will be reimbursed to the quota specified in your insurance policy. Where applicable, your co-payment will be collected at the time of your visit. Your insurance may cover most but not all of your expenses. Please check with your insurance provider before scheduling an appointment.
If your insurance does not appear on the above list, we are considered “Out of Network” by your insurance. We do offer out of pocket treatment based on income. Please reach out to our office to find out more.
We offer an out-of-pocket (self) payment option. Please call our office to discuss accepted forms of payment.
You can call to cancel or reschedule your appointment up to 24 hours prior to your appointment time? Failure to do so will result in a $75 “no show” charge applied to your account.
We do our best to accommodate our patients in case of unforeseen situations. However, if you arrive more than 30 minutes late for a first encounter, your appointment will be rescheduled. If you arrive more than 15 minutes late for a follow-up, your appointment will be rescheduled. Our providers need enough time to care for our patients appropriately.
